Please complete the below form to apply for the Dawson Creek Arts and Wellness Fund. Applications are ongoing and forms will be reviewed as soon as possible. Please ensure that you apply at least one week before the program start date.
Completed forms can be emailled to us or printed and dropped off at Calvin Kruk Centre for the Arts (KPAC), City Hall Recreation Services, Dawson Creek Art Gallery, The Ark Youth Centre (attn. Laurelle Allen) or Nawican Friendship Centre.
